Bidder does not have an independent formal experience
modification rating from the Workers Compensation
Insurance Rating Bureau for any of the years required
in Form 200 and Form 450, Bidder must submit written documentation from its workers
compensation insurance company or its licensed and
certified broker establishing, to the
satisfaction of the County of Sonoma’s Risk
Management Director, that Bidder’s independent
experience modification rate is equivalent to the
rates established in Form 200 and Form 450.

sheets A1 and A2.

Metal Roofing Finish and Trim Gauge Clarification:
Change section 07610 2.2A4 to Zatique finish applied
to a single side of the roof material.
Change details 3, 4, and 6 on Sheet A3 flashing and
gutters from 20 gauge to 22 gauge.

in Volume 1 of 3) acknowledging receipt of this
