Health Access Programs for Uninsured or Underinsured Patients

Family PACT
Confidential family planning services for women and men determined eligible on-site with participating Medi-Cal providers.

Every Woman Counts
Free breast and cervical cancer screening and treatment for eligible women. Call 1-800-511-2300 for more information.

BabyCAL
Women without health insurance should consider applying for Medi-Cal if they become pregnant. Eligibility requirements are less restrictive for pregnancy-only coverage. Sonoma County residents may apply for Medi-Cal at the Department of Human Services, 2550 Paulin Drive, Santa Rosa or at a location that provides Medi-Cal application assistance. Women who think they might be eligible are encouraged to contact a provider participating in the Medi-Cal Presumptive Eligibility (PE) for Pregnant Women. This program issues women a temporary card to begin their prenatal care prior to completing the formal Medi-Cal application process.

Comprehensive Perinatal Services Program (CPSP)
Nutritional, psychosocial and health education support services are available to perinatal patients through the Comprehensive Perinatal Services Program. Qualified providers may receive reimbursement from Medi-Cal for delivering these enhanced obstetrical services.

Presumptive Eligibility (PE) for Pregnant Women Program
Allows Qualified Providers to grant immediate, temporary Medi-Cal coverage for ambulatory prenatal care and prescription drugs for conditions related to pregnancy to low-income, pregnant patients, pending their formal Medi-Cal application.

Access for Infants and Mothers logoAccess for Infants and Mothers (AIM)
Women that are uninsured or underinsured and do not meet income guidelines for Medi-Cal may qualify for the AIM Program. AIM offers affordable prenatal and maternity insurance for middle-income pregnant women, and health insurance for their babies through Healthy Families program.

Maternal Child Health Field Nursing
Experienced Public Health Nurses make free home visits to pregnant women and families with young children at risk.

Prenatal Care Missed Appointment Program (PC-MAP)
If your prenatal care patient fails to keep her scheduled appointment, you can make a referral to Maternal Child Health Field Nursing. Bilingual outreach staff will contact the women by phone, or make a home visit to assess factors that might be preventing her from coming to her prenatal visits. Confidential case management services to support pregnant and parenting teens and young adult fathers.

Families First Home Visiting
Free postpartum home visits for all first-time parents in Sonoma County.

Smoke Free Babies

Alcohol and Other Drug Recovery Resource Guide for Women (pdf)
A list of local gender-specific outpatient and residential treatment programs available from various local agencies. If a pregnant patient at risk for alcohol or other drug use is ready to discuss treatment, providers may contact Sharon Youney, the Perinatal Placement Specialist with Sonoma County Alcohol and Other Drug Services for assistance. She is available Monday through Friday at 236-2362 (cell phone).

Bereavement Services

Sonoma County SIDS Program
Public health nurses, specially trained in bereavement, provide grief support, information, referrals, and link families and caregivers affected by SIDS to peer support programs.

California SIDS Program
Includes grief and bereavement resources, SIDS facts, and infant care practices for reducing the risk of SIDS.
