Services: The Center for HIV Prevention and Care of the County of Sonoma, Department of Health Services, Public Health Division, is a resource for HIV medical expertise in Northern California and helps clients and local medical providers keep abreast of the latest developments in HIV treatments. Services: A team of 6 physicians provides HIV primary medical care at the Santa Rosa facility. Coordination and consultation on the medical, social, and psychological needs of HIV-positive patients are provided by a diverse team of specialists, including: Social Workers, Nutritionists, Nurses, Health Educators, Pharmacists, Benefits Counselors, Discharge Planner, Psychologist and HIV Specialty Physicians.
