Community Health Services
Community Health Services involves those public health programs aimed at prevention of disease and promotion of health and seeks to provide those services in communities, where most eligible clients live. The following programs are administered by CHS:
California Children Services (CCS)
If your child has a serious medical condition, our California Children Services program may be able to provide financial assistance for diagnostic and treatment up to the age of 21 years.
CCS Medical Therapy Unit (MTU)
Children with physical handicaps who are eligible for the CCS program may receive physical and/or occupational therapy through the Medical Therapy Unit. The goal of therapy is directed toward attaining maximum function within the limits of the child's capabilities.
Child Health and Disability Prevention (CHDP)
This preventive program makes early health care available to children and youth with health problems as well as to those who seem well. Eligible children and youth receive periodic preventive health assessments, and those with suspected problems are then referred for diagnosis and treatment.
Childhood Lead Poisoning Prevention (CLPP)
Keeping children safe from lead poisoning requires attention from everyone in a child's life. The CLPP program employs a team of health professionals, who provide services to parents, healthcare providers, and the general public.
Families First Home Visiting
A program for every Sonoma County parent having their first baby. Three home visits, information about community resources, and gifts for your baby's health and safety - all to assist you in your new role as a parent.
Maternal, Child, Adolescent Health
By coordinating perinatal services for the families, and providing outreach and education services to the local community, MCAH helps families obtain access to pregnancy and prenatal care. MCAH also provides health and safety consultation for child care providers.
Maternal Child Health Field Nursing
The Public Health Nurses work to help families find services for family planning, pregnancy care, child health care and food.
Public Health Clinics
Our Public Health Clinics offer access to many testing and treatment services, including STD testing and treatment, pregnancy testing and counseling, birth control, immunizations, and services for teens.
Teen Parent Connections
If you are 18 and under and you're pregnant or already have a child, Teen Parent Connections can help you have a healthy baby, stay in school, budget your money, plan a career, and be the best parent you can be.
Women, Infants and Children (WIC)
The WIC nutrition program helps mothers and young children eat well and stay healthy.
